Otis E. “Jack” Holtsclaw 50 of Hopkinsville, KY and formerly of Breese died Wednesday November 17, 2010 at Jennie-Stuart Medical Center in Hopkinsville, KY 

He was born December 31, 1959 in Shawnee, OK to JoeAnn (Coffin) Woldark: his mother survives in Breese 

Preceded in death by his step-father Charles Woldark and father-in-law George Kreiter 

He married Theresa Kreiter October 26, 1983 in Edwardsville and she survives in Hopkinsville, KY 

Also surviving are sons Brandon Holtsclaw and Eric Holtsclaw both of Kansas City, MO  

7 grandchildren

Funeral service Monday November 22, 2010 at 10:00 am at Moss Funeral Home in Breese with Deacon Bob Lippert officiating. Interment Elwood Cemetery, Breese 

Visitation will be Sunday November 21 from 5 to 8 PM and Monday November 22 from 8 to time of service at the Moss Funeral Home in Breese
